St. John taking preschool applications

RESERVE – The St. John the Baptist Parish School Board is now accepting applications for the 4-year-old preschool programs for the 2013-14 school year.
There are four programs in the parish.
A child must be 4 years old by Sept. 30, 2013, to be eligible. Every child will qualify for one of the preschool programs regardless of income.
Applying early is recommended. Appli-cations should be submitted in person at one of the following ad-dresses: The St. John Child Development Center at 117 Stebbins St., Garyville LA 70051 or West St. John Elementary, 2555 Lou-isiana Highway 18, Edgard, LA 70049.
When applying, the following is also needed:
• Income verification – Family income must be verified before a child’s eligibility for Head Start can be determined. Any of the following documents can be used to determine income: income tax form 1040 for the previous calendar year; W-2 statements for all working family members for the previous calendar year; pay stubs (recent) for all working family members; documentation showing current status/amount received from SSI/Social Security; foster care documentation letter for foster child; documentation of child support/alimony; documentation of unemployment compensation; written statement from current employer(s); written statement of family support from person supporting the child; or income declaration form.
• Child’s birth certificate to verify age of child
• Child’s Social Security card
• Child’s medical insurance or Medicaid card
• Documentation of child’s disability (IEP or IFSP), if applicable
• Child’s immunization record to show that the child is current on all shots
Before a child can attend Head Start, the child will need a Head Start physical examination form signed by the child’s physician or clinic provider to show that the child is free from communicable disease.
